On March 24, 2003, VVRH opened its new Quick Care Unit. With this addition, all emergency room patients, regardless of their need, benefit from less waiting time and speedier service.

Does Valley View Have Authorization to Treat Your Child in Your Absence?

According to Oklahoma law, minor children must have parent or legal guardian consent for medical treatment. Except in a life-threatening situation, treatment could not be administered without it. Persons entrusted with the care of your child cannot give consent for treatment unless legally authorized by a parent or guardian.

Click here for a printout of the Authorization for Medical Care of a Minor consent form. We have provided this form in a PDF file to make it easy to print out. Click here to download Adobe Acrobat Reader...

This form provides Valley View Regional Hospital legal authorization for medical and/or dental treatment and insures that your child will receive treatment without delay.

Please complete the form and mail it to: ATTN: Emergency Room, Valley View Regional Hospital, 430 N. Monta Vista, Ada, OK  74820. You may want to make a copy and keep it in your files at home.
