Your Medical Record
Your medical record is the physical property of the healthcare practitioner or facility who compiles it. Providers of care must keep records to document the services they render. They follow a number of safeguards to maintain confidentiality.
If a patient is a competent adult, only the patient may consent to the release of the records. If the patient is deceased, a personal representative appointed by the probate court may authorize release. If no personal representative exists, the patient's next of kin may authorize release.
