Social Work

A stay in the hospital can be stressful both physically and mentally, as well as financially.  Valley View's Social Work department eases your burdens by identifying specific needs and then assisting you and your family with resources to satisfy those needs. Our goal is to assist you during your treatment with information, personal support and referrals for services you may need outside the confines of the hospital.

Social Work services are provided to all inpatient areas of the hospital, as well as out-patient areas such as Dialysis, Oncology,  Emergency Room, One Day Surgery and Home Health Care.

The need for social work services is determined by a Needs Assessment performed during the Pre-Admission process into the hospital. A carefully designed Discharge Plan is set in place before you leave.

Some of the services provided include:

  • Supportive Counseling

  • Support Groups

  • Linkage to Community Resources: Medical Equipment, Home Health Care, Home Delivered Meals, DHS Entitlement; Caregiver Resources, Advance Directives

  • Liaison for Family Members

  • Crisis Intervention: Child Abuse/Neglect, Adult Abuse/Neglect, Mental Health,  Domestic Violence, Sexual Assault, Sudden Death, Major Trauma, Chemical Dependency

  • Hospice Program
